Community Partners
Hilltop Community Health Center—Nonprofit clinic dedicated to helping Northwest Indiana residents receive primary care services, regardless of ability to pay.
Indiana Association of School Principals—Advocates for and supports the vision component in their students' education and health.
Indiana Minority Health Coalition—Develops initiatives to address the health disparities of ethnic and racial minority populations, including vision care for at-risk populations.

IU Health
Lions Clubs—Provide vision-screening services to area schools and contribute monetary assistance for eyewear not covered by our in-kind doctor network.
Marion County Department of Health—MCHD Nurses are trained to screen students and work with school nurses to obtain referral and follow-up care information.
Northwest Indiana Community Action Corporation—Nonprofit that provides assistance to low-income, disabled and elderly families in Northwest Indiana.
Office of Women's Health—Improves health status of women in Indiana across the life span through assessment of health needs, increased public awareness and education and coordinated development of women's health programming. One such program is the Diabetes Patient Navigators, a service of which is vision care in three pilot counties through PBI's in-kind doctor network.
Project Health—Provides eye surgeries and comprehensive health care to uninsured Marion County residents between 200 to 300 percent of poverty level.
RSVP—Provides vision screenings and locates follow-up services for students in Northwest and Southeast Indiana.
Reach Out and Read—Promotes early literacy by giving new books to children and advice to parents about the importance of reading aloud via pediatric exam rooms across Indiana and the U.S.
Ruth Lilly Health Education Center—Indiana largest health education center empowers students to make better informed decisions about their health and well-being.
EYECARE AMERICA ONLINE REFERRAL CENTER
Additional resources are offered by EyeCare America to US citizens or
legal residents who are without an EyeMD, who do not belong to an HMO,
or have eye care through Veteran's Administration, and qualify in one of
the followign ways:
1. Those who are 65 or older and have not seen an Ophthalmologist (EyeMD) in three of more years. These
patients may be eligable to receive a comprehensive, medical exam and up to one year of care at no out-of-pocket
cost for any disease diagnosed during the initial exam.
2. Those determined to be at increased risk for glaucoma (by age, race, and family history) and have not had an eye
exam in 12 months or more. These patients may be eligable to receive a free glaucoma eye exam if they are
uninsured.
